The links below will allow you to download and print the blank forms required by Alabama Law, Title 34, Chapter 14 and Alabama Code 445 to be completed in order to either apply for a license in the State of Alabama or to renew your license to dispense hearing instruments within the State of Alabama. You must print, complete and submit the original forms to the following address:
